Join us on Thursday, August 7 from 12:30 – 2:00 PM at Potomac Place for an educational session focused on Medicare, dementia support, and home health — featuring special guest Tyler Lloyd, with Five Star Home Health Care.

Tyler will share insights on the GUIDE Model, which provides comprehensive, coordinated dementia care aimed at improving the quality of life for people living with dementia. This model also reduces the strain on unpaid caregivers and supports individuals in staying safely in their homes and communities. It includes Medicare-covered care coordination, caregiver education and support, and respite services.
